Q: What is J2 Care Collective?

J2 Care  Collective is a care coordination and patient advocacy service. We help  families and individuals navigate the healthcare system — from scheduling  appointments and communicating with providers to understanding diagnoses and  managing care transitions. Think of us as a dedicated partner who handles the  logistics so you can focus on what matters most.

Q: Are you a medical provider?

No. J2 Care  Collective is not a medical practice and we do not provide clinical care,  medical advice, or treatment. We are care coordinators and patient advocates.  We help you communicate with your providers, understand your options, and  organize your care. All medical decisions remain with you, your family, and  your healthcare team.

Q: Who do you work with?

We work with  family caregivers supporting a loved one near or far, individuals managing  their own complex or chronic care, seniors navigating the healthcare system,  families facing a new diagnosis or major care transition, and anyone who  feels overwhelmed or lost in the healthcare system. If you're not sure  whether we're the right fit, reach out. We're happy to talk it through.

Q: Where are you located? Do you work remotely?

We are based  in the San Francisco Bay Area (Vallejo/Fairfield specifically), but work with  clients virtually across California. Many of our services — including  appointment accompaniment, provider communication, and check-ins — can be  done remotely. For clients in the Bay Area, in-person support is also  available depending on the service tier.

Q: What makes J2 different from a home health agency or medical social  worker?

Home health  agencies provide hands-on clinical care. Medical social workers support  patients within a specific hospital stay. J2 is different: we’re independent,  non-clinical, and focused on coordination and communication across all your  providers — over time, not just during one appointment or hospitalization. We  work for you.

Our Services

Q: What do you actually do?

We handle the  healthcare logistics that take up so much time and mental energy. This  includes coordinating appointments and referrals, attending appointments with  you (virtually or in person), assisting with follow-ups, communicating with  doctors and other providers on your behalf, creating and maintaining a care  plan, supporting hospital discharges, and keeping families informed and  aligned. The specifics depend on your situation and the support tier you  choose.

Q: What's the difference between your service tiers?

We offer  three monthly tiers and an hourly option. Tier 1 (Essential) is best for  light, occasional guidance. Tier 2 (Active Coordination) works well when  you're managing multiple providers or care transitions. Tier 3 (Intensive) is  for high-need or complex situations that require more hands-on support. All  packages are month-to-month with no long-term commitment. Reach out for a  free consultation and we’ll walk you through pricing.

Q: What's NOT included?

We do not  provide medical advice, clinical care, or emergency support. We do not offer  after-hours phone support or crisis management. We do not provide in-person  transportation. For anything outside our scope, we will connect you with the  right resources or providers.
